How Can You Participate?

New Hires have 90 days from date of hire to enroll in the hospital indemnity insurance. Eligible dependents include spouses and unmarried children or stepchildren, under age 25.

What Is An Indemnity Benefit?

It means that the insurance company will pay a set amount each time the insured receives a covered service. The same amount is paid regardless of the fees charged by the provider.

When will my coverage begin?

Your coverage will be effective on the first Monday after your payroll deductions begin.

How do I pay for my benefits?

You will pay through payroll deductions. Payroll deductions will automatically be set up when you enroll. If you miss a payroll deduction as a result of absence or lack of work, no benefits will be extended for the following period. However, if a claim is incurred during a period of time when premiums were missed, the missed premium will be subtracted from the pended claim, maintaining a continuity of coverage. If a claim is not incurred during the missed premium timeframe, missed premium does not need to be made up. However, if you miss 5 consecutive weeks of premium, coverage will be terminated and you will not be eligible to re-enroll until the next open enrollment period unless you experience a qualifying event.

Can I drop this coverage later?

Yes. However, you will not be eligible to re-enroll until the next open enrollment unless you experience a qualifying event.

Will I get ID cards?

For each plan chosen, an ID card will be mailed approximately 3 weeks after your first deduction. If you need to request a new ID card, please use the contact numbers below.

What happens if I am almost at, or I have exceeded, a medical plan limit and I need to see a doctor?

You are responsible for any amount owed above your plan limit.
