Limited Medical
Limited medical is a fixed-benefit plan that pays set dollar amounts toward everyday care like doctor visits, lab tests, X-rays, ER visits, surgery, and hospital stays. It is not full major medical insurance, so any cost above those set amounts is paid by you.
Dental
Preventive care (exams, cleanings, X-rays) is covered at 100% in-network with a $20 deductible per visit, or 80% out-of-network, with no waiting period. Basic care (fillings, root canals, gum treatment, extractions) is covered at 60% after a 3-month waiting period, and major care (crowns, onlays, dentures/bridges) at 50% after 12 months. The annual maximum is $500 in-network and $1,500 out-of-network, and the out-of-network deductible is $50 per year.
Vision
In-network, eye exams and standard lenses are covered in full after a $10 exam copay and a $25 copay for lenses or frames. Frames and elective contacts get a $105–$130 allowance. Out-of-network pays set amounts, like $45 for an exam and $70 for frames. You get an exam and lenses every 12 months and frames every 24 months, plus extra VSP discounts on glasses and laser vision correction.
Accident
Voluntary accident insurance provides a range of fixed lumpsum benefits for injuries resulting from a covered accident. This plan offers coverage for accidents, injuries, ambulance services, and accidental death in addition to your primary medical insurance. When you or a covered family member has an Accident, you may seek treatment from a physician, urgent care, or hospital.
